Three jobs a topical can do
Almost every foot-pain topical does one of three things, and the whole trick is matching the job to the problem:
- Mask the pain signal. Menthol and lidocaine both do this, in different ways. They change what your brain receives without touching the underlying injury.
- Calm the inflammation. Topical NSAIDs reduce the actual inflammatory process driving the pain.
- Quiet an overactive pain nerve. Capsaicin gradually turns down nerves that are firing pain when they should not, which is why it is the one that shows up for nerve pain.
Menthol (Biofreeze, Icy Hot): fast and temporary
Menthol is a counterirritant. It activates a cold-sensing skin receptor called TRPM8 and floods your nervous system with a cooling sensation that crowds out the slower pain signal (the classic gate control theory of pain). It does not touch inflammation or the injury itself. The profile: relief in minutes, gone in an hour or two, purely symptomatic. A very good "quiet this down right now" tool, and a poor "fix it" tool.
Reach for it: everyday soreness, tired achy feet, a symptom you want softened before or after activity.
Topical NSAIDs (Voltaren, diclofenac): treats the inflammation
Topical diclofenac is a real anti-inflammatory, the same drug family as ibuprofen, delivered through the skin. It blocks the COX enzymes that produce inflammatory prostaglandins, so it works on the process rather than the sensation. It works gradually over days of consistent use, not on the spot. Pooled Cochrane data found roughly 6 in 10 people got meaningful relief over 6 to 12 weeks. It has been available over the counter in the US since 2020, and because it concentrates locally, it puts far less drug in your system than oral ibuprofen (far less, not none).
Reach for it: chronic, inflammatory problems near the surface, like Achilles tendinitis, plantar fasciitis, or arthritis of the big toe joint.
Lidocaine (patches, creams): numbs a specific spot
Lidocaine is a local anesthetic. It blocks the sodium channels that nerves use to fire, quieting the aberrant pain signals from the nerves directly under the patch. The 4% patch is OTC; the 5% patch is prescription and is FDA-approved for the nerve pain of shingles (postherpetic neuralgia). Very little is absorbed into the body, and you generally do not feel numb, you feel less pain. It works best when the pain is well-localized to one spot you can cover with the patch, rather than spread across the whole foot.
Reach for it: a discrete, pinpointable painful area, or localized nerve pain. Less useful for diffuse, all-over foot pain.
Capsaicin (creams, 8% patch): retrains the pain nerves
Capsaicin, the compound that makes chili peppers hot, is the most interesting one for foot specialists. It activates a receptor called TRPV1 on pain nerves, and with repeated exposure it "defunctionalizes" those overactive fibers, effectively turning down the volume on chronic nerve pain over time. Low-dose creams are OTC but demand consistent daily use and cause a burning sensation at first that fades. The high-concentration 8% patch (Qutenza) is applied in a clinic and is FDA-approved for diabetic nerve pain in the feet and for postherpetic neuralgia. Among topicals, capsaicin and lidocaine have the strongest evidence for neuropathic pain, well ahead of menthol.
Reach for it: burning, tingling neuropathic foot pain, especially from diabetic neuropathy. This is a conversation to have with your clinician, particularly for the in-office 8% patch.
Salicylates (Aspercreme, some Bengay): the aspirin cousins
Methyl salicylate is a topical relative of aspirin, found in products like Aspercreme and some Bengay formulas. It works partly as a counterirritant (a warming or cooling sensation) and partly as a mild salicylate. For most people it is a matter of preferred feel versus menthol rather than a meaningfully different result. One caution worth flagging: menthol-plus-salicylate combinations are the products most associated with the rare skin burns described in the safety section below.
The "natural" aisle: arnica and CBD
These get asked about constantly, so here is the straight answer. Arnica gel has some evidence, one reasonable study found it comparable to an NSAID gel for hand osteoarthritis, but the overall quality of research is low, so call it "possibly mildly helpful, not proven." CBD topicals are enormously popular, but the evidence is still preliminary, and, just as importantly, OTC CBD products are not held to standardized dosing or quality regulation, so what the label claims is not guaranteed to be what is in the tube. That lack of standardization is a big reason they cannot replace treatments with real clinical data behind them. Neither arnica nor CBD is harmful for most people, and if one brings you comfort, there is little downside. Just do not let a natural cream delay care for a foot problem that needs actual treatment.
Which topical for which foot problem
This is where the guide earns its keep. Matched to the conditions I see most:
- Plantar fasciitis and Achilles tendinitis: a topical NSAID between sessions to work on the inflammation, with menthol for on-the-spot symptom relief. Neither replaces stretching, load management, and the rest of the plan.
- Arthritis: big toe joint or midfoot: topical NSAID is the best-supported topical here, since these joints sit near the surface.
- Burning, neuropathic feet (diabetic neuropathy): capsaicin has the strongest topical evidence, with lidocaine useful for a localized hot spot. Menthol may give brief comfort but is not therapy. See the neuropathy cautions below before applying anything.
- Morton's neuroma: a lidocaine patch or NSAID may take the edge off, but a neuroma is a mechanical nerve problem that usually needs footwear changes, padding, or in-office treatment, not a cream alone.
- Gout flare: skip the gels. An acute gout attack is an inflammatory storm that needs prompt systemic treatment, not a topical.
- Everyday post-activity soreness: menthol, for exactly the speed-and-simplicity reason it exists.
The safety rules that matter
Do not mask an injury to keep going
Pain during activity is often a stop signal. Numbing it to push through is how a strained tendon becomes a full rupture and a stress reaction becomes a stress fracture. Use topicals for ordinary soreness, not to silence a pain that is sharp, worsening, or tied to a specific movement.
If you have neuropathy, diabetes, or poor circulation
- Look before you apply. Check the whole foot in good light, including between the toes. Never apply any topical over an open wound, blister, crack, or ulcer. Neuropathy can hide the damage from you.
- Never add heat. The FDA has documented serious burns from OTC menthol and salicylate products, and adding a heating pad or hot water bottle over them has caused full-thickness burns, one published case went from a topical burn to a below-knee amputation. Menthol scrambles temperature sense, and neuropathy already has, so you can injure the skin without feeling it. No heat, and no tight bandaging.
- Apply gently. Fragile, neuropathic skin does not need vigorous rubbing.
Product-specific cautions
- Capsaicin: wash your hands thoroughly after applying and keep it well away from eyes, nose, and other sensitive areas, it burns badly on contact. The mild initial burning at the site is expected and fades with continued use.
- Topical NSAIDs: they still put some drug into your system. If oral NSAIDs are off-limits for you (kidney disease, certain heart conditions, ulcers, NSAID allergy), do not assume the gel is automatically safe, and do not stack it on top of oral ibuprofen. Generally avoided in the third trimester of pregnancy.
- Lidocaine patches: follow the label on how many patches and how many hours; more is not better.
The bottom line
There is no single best topical, only the right tool for the specific problem. Menthol quiets soreness fast. Topical NSAIDs treat inflammation. Lidocaine numbs a spot. Capsaicin is the one for nerve pain. Match the mechanism to what is actually wrong, and treat all of them as helpers alongside the real fix, not as the fix itself. Anything sharp, sudden, red-hot, spreading, or refusing to settle deserves a clinician rather than another layer of gel.
